Job Summary

We are seeking a compassionate and dedicated Disability Home Care Social Worker to join our team. In this role, you will be responsible for providing essential support and assistance to individuals with disabilities in a home care setting. Your primary goal will be to enhance their quality of life and promote their independence through personalized care and social services.

Responsibilities

  • 1.Conduct comprehensive assessments of individuals with disabilities to identify their unique needs and preferences.
  • 2.Develop individualized care plans in collaboration with clients, their families, and interdisciplinary teams.
  • 3.Provide counseling and emotional support to clients and their families, addressing issues such as adjustment to disability, coping mechanisms, and advocacy.
  • 4.Coordinate and facilitate access to community resources and services, including medical care, therapy, vocational training, and recreational activities.
  • 5.Advocate for clients’ rights and entitlements, ensuring they receive appropriate benefits and services.
  • 6.Monitor clients’ progress and reassess needs regularly, adjusting care plans as necessary.
  • 7.Document all client interactions, assessments, and interventions accurately and timely.

Requirements

  • 1.Bachelor’s or Master’s degree in Social Work or a related field.
  • 2.State licensure or certification as a Social Worker (LCSW or equivalent).
  • 3.Proven experience working with individuals with disabilities, preferably in a home care or community-based setting.
  • 4.Strong knowledge of disability rights, legislation, and available support services.
  • 5.Excellent communication and interpersonal skills, with the ability to establish rapport with clients from diverse backgrounds.
  • 6.Ability to work independently and as part of a multidisciplinary team.
  • 7.Empathy, patience, and a genuine desire to improve the lives of individuals with disabilities.
